  • Common Carrier Benefit:  The above payment schedule is DOUBLED for a covered loss occurring from accidents resulting while Insured Person is in, entering or exiting a Common Carrier.

  • Extensions of Insurance:   Exposure - If a covered Accident causes an Insured Person to be unavoidably exposed to the elements and as a result of such exposure an Insured Person has a Loss, then such Loss will be insured under this policy. Disappearance – If an Insured Person has not been found within one year of the disappearance, stranding, sinking, or wrecking of any Conveyance in which an Insured Person was an occupant at the time of the Accident, then it will be assumed, subject to all other terms and conditions of the policy, that an Insured Person has suffered Loss of Life insured under the policy.

    In-hospital Accident Insurance:
    The Insurance Plan for ELITE members described above includes an enhanced in-hospital benefit which pays $100 per day ($200 daily for ICU) after 3 days of hospital confinement. This benefit is payable for up to 5 consecutive days.

  • Both of the above coverages become effective on the 1st day of the second month following the effective date of your membership. (Example: If your membership effective date is September 30, your coverage becomes effective on November 1).
  • There is nothing you need to do in order to receive the above coverages.
  • The cost is included in your FACT dues.

  • How to File a Claim:
  • To obtain a claim form go to the carrier’s website (www.chubb.com). Click on “Report a Loss," then click on “Accident and Health," and then select the appropriate claim form and print.
