FACT ELITE members may complete a free electronic health record (EHR). An EHR stores all of your personal health information in one secure place that is accessible to you 24 hours a day, 365 days a year. You control who can see the information in your record and can easily share it with your primary care physician or other health professionals.

How to Take Advantage:
This benefit is made available through UnitedHealth Allies, which will send you an ID Card allowing you to access this benefit. If you do not receive this card within 30 days after joining FACT, please call 1-800-USA-FACT or e-mail: cservice@usafact.org
