Hospitalization Fund
$1,000 to $5,000 may be payable for hospitalization due to a major medical problem (heart attack, stroke, major organ transplant, cancer, kidney failure). This is NOT insurance -- it is instead a fund maintained exclusively for FACT members to provide help in addition to any insurance the member may own. There are no long claim forms to complete ... no need to meet any underwriting standards ... no difficult-to-understand language to wade through.

The premium is paid by FACT! Includes payment for loss of limb, hearing or sight. Covers member's spouse and all dependent children. Pays double for a common carrier accident.

$250 Lump-sum Ambulance Benefit
In the event of an emergency, an eligible FACT member can receive up to $250 for actual expenses incurred for the use of ground ambulance service. Reimbursement includes one trip per membership year per household.
