In-Hospital Crisis Care Fund

This Fund exists to give FACT ELITE members almost instant cash -- to be used in any manner they choose -- if they are hospitalized due to one of many major medical problems. The benefit is available for the ELITE Member, his or her spouse, and all dependent children living in the same household.
The Fund is NOT insurance, so there are no long claim forms to complete ... no need to meet any underwriting standards ... no difficult-to-understand language to wade through. Quite simply: A stricken member submits his or her request with a copy of a physician’s diagnosis and proof of hospitalization due to that diagnosis. The request is immediately reviewed and, if certain criteria are met and the Fund has not been exhausted, a lump-sum payment is made within a matter of days (if not hours!). 

In general, the following serious conditions qualify for help:
• Heart Attack
• Stroke (not including T.I.A. mini-stroke)
• Major Organ Transplant
• Bone Marrow Transplant
• Cancer
• Kidney Failure

The lump-sum payment is made in addition to any insurance you may have for your first hospitalization due to one of the above conditions.
• For a hospitalization of more than 3 days, the payment is: $1,000
• For a hospitalization of 4 through 14 days, the payment is: $2,500
• For a hospitalization of 15 days or more, the payment is: $5,000

There can be no more than one Fund payment to any FACT household in any given 24-month period, and no more than one Fund payment to any member of a FACT household for the same condition. If two or more requests are received from members who are being diagnosed and treated by the same physician, those requests may be subject to additional review. Monies from the Fund are available only for those conditions which are diagnosed 61 or more days after the effective date of the requester’s FACT ELITE membership.

The In-Hospital Crisis Care Fund has been established as an altruistic vehicle for FACT ELITE members only. It is administered by the Council for Disaster Assistance, Inc., (CDA), a not-for-profit corporation. All requests will receive serious and immediate consideration. However, CDA sets the terms and conditions for Fund withdrawals, and all requests for payments are subject to that organization’s review and adjudication.
