Long-Term Care Services

FACT ELITE members receive discounts of 5% to 30% on a variety of long-term care services from participating providers. Services include:
Adult Day Care: Daytime-only community-based programs that provide a variety of health, social, and other support services in a
- protective setting.
Hospice Programs: Programs that provide comfort and care for terminally ill patients, either in a special facility or at home, including pain
- management, emotional and spiritual support to the terminally ill and their families.
Home Health Aides: In-home assistance with activities of daily living such as bathing, dressing, and mobility

Respite Programs: Programs that provide temporary care for elderly or disabled persons, providing some time off for the primary
- caregiver. Respite care can be offered in a nursing home, at an adult day care center or other facility, or at home.
Homemaker and Personal Care Services: In-home help with meal preparation, shopping, light housekeeping, money management,
- personal hygiene and grooming, and laundry.
Skilled Nursing Facilities: Discounts of 5% to 15% at facilities that provide care for persons whose physical needs require extensive help
- from nurses, aides, or physical therapists.
Assisted Living: Save 5% at facilities that provide a more home-like environment than that of a nursing home, at about half the cost, for
- people who need help with activities of daily living. Most facilities combine personal living space with areas for social activities.
